The following is a brief list of application materials.

A. Application Form

B. Application fee of US $30.00 (non-refundable, check or money order payable to O.T.S.)

C. One 2-inch recent photograph.

D. Personal testimony that should have two portions:

1. A statement of your past Christian experience, including your conversion, subsequent commitments, significant spiritual events, and

formative influence in your life.

2. A statement describing and evaluating your present spiritual life and Christian service, including your involvement in church and/or Christian

organizations,areas in which you are experiencing growth, details concerning your call and decision to enter some form of Christian ministry,

and your gaols for the future.

E. Two recommendation letters. One must be from the minister (pastor or elder) of your current church. Please fill out the top portion of OTS

Recommendation Forms and send them to your recommenders. Ask your recommender to send the completed forms directly to the OTS

Admissions Office.

F. One notarized copy of graduation certificate of your highest degree.

TOEFL scores is to be send from Educational Testing Service directly to OTS.

G. Official transcripts mailed directly to OTS from the schools you attended post high school (technical school, college/university, graduate school,

Bible school).

H. Due to space limitation, on-campus housing is only available to full-time students. Those applying for housing must submit the OTS Application for On-Campus Housing Form and the Health History Form to the Admission Office. It is advisable for families with young children to seek off

campus housing.

I. International students must submit proof of sufficient funds for the first year of study at OTS for himself/herself and his/her family before a I-20

can be issued. All financial related document must be original.
